Students admitted to kindergarten must have attained age five (5) by the 30th day of September of the current school year.
Students admitted to the first grade must have attained the age of six (6) years by the 30th day of September of the current school year. In addition, the student must have attended an accredited kindergarten.
All students must comply with current State of Ohio immunization and health requirements before enrollment.
Process
At the time of initial application, please present the following to the School office:
Application Form
$150.00 + $2.00 Application Fee (Non-Refundable)
Birth Certificate (original or copy)
For students applying for first through eighth grades:
Copy of report cards
Copy of standardized test results (if applicable)
Copy of IEP/504/Service plan (if applicable)
Custodial Documentation – A certified copy of any child custody agreement, order or decree pertaining to the child must be presented, if applicable.
All students are tested before being admitted to Saint Agatha School. Parents will receive more information about dates and times for their child’s testing/kindergarten screening. Within 48 hours of your child’s testing/kindergarten screening, you will receive an admission decision email.
Acceptance
Upon acceptance to Saint Agatha School, each student is required to return the signed admission decision letter and a $150 enrollment fee within two weeks of receipt of acceptance to secure a spot.
Due upon acceptance:
Baptismal Certificate – Original or copy must be provided only if child was not baptized at Saint Agatha Parish.
Birth Certificate – Original or copy must be provided.
$150 Enrollment Fee per Child : Please note that the deposit is non-refundable and will be applied to the first tuition payment of the new school year.
Signed admission decision letter.
